Health Related Home Care: Designated Cardiovascular Pioneer

Health Related Home Care: Designated Cardiovascular Pioneer

Health Related Home Care was recently named a “Cardiovascular Pioneer” by the Home Health Quality Improvement (HHQI) National Campaign, which is an initiative of the federal Centers for Medicare & Medicaid Services.

The Pioneer designation is awarded to home health agencies that make a commitment to helping patients lower their risks for heart attacks and strokes.  Participating agencies implement patient-centered programs to improve aspirin therapy, blood pressure control, cholesterol management and smoking cessation (known as the ABCS of cardiovascular prevention).  They also agree to report statistics to the HHQI campaign, which tracks progress locally and across the nation through its Cardiovascular Data Registry pilot project.

Sharon Norryce said Health Related Home Care is participating in the quality improvement and reporting initiative because the health and well being of patients is its number one priority.

“We are committed to high quality care for our patients and are pleased to be helping the HHQI National Campaign prevent heart attacks and strokes,” said Mrs. Norryce. “As a Cardiovascular Pioneer, we will also be measuring our progress and helping contribute to better care for patients, better health for our communities, and lower costs through improvement of the health care system.”

Health Related has been recognized as Home Care Elite for 6 out of 8 years, which compares agencies ability to improve pt. outcomes and high satisfaction scores across the nation, thus making HRHC in the top 7% of the most successful home care agencies across the county. HRHC works on specific care pathways based on diagnosis, to improve patients. ability to remain comfortably at home.

Since 2007, the Home Health Quality Improvement (HHQI) National Campaign has been dedicated to improving the quality of care provided to America’s home health patients with evidence-based tools, timely data reports and a wealth of ongoing educational opportunities for home health agencies.
